They do rule cities, they just don’t rule them directly, because Westeros is a feudal society so power is usually distributed rather than centralized.
Don’t you find it a little odd that none of the Great Houses of Westeros rule a city. Lannisport is run by a cadet Lannister branch which is technically a separate House.
