It depends on both time (during the time of Alfred the Great, West Saxon custom was that the wife of a king was not allowed to be titled as Queen, nor sit a throne alongside her husband; by the present day, English common law says that the wife of the king is automatically the Queen) and place.
The political difference would be minimal in the present day, but historically could have had a great influence on whether the wife of a king would become regent upon the king’s death and similar matters.
