Given that Moat Caitlin, despite being North’s greatest defence, is very vulnerable to a rapid amphibious strike from the rear up the Fever/Saltspear , why isn’t there a 2nd Wolf’s Den designed to cut off the Ironborn from sailing up those rivers?

This is the big mystery to me. At the very least, you’d expect a boom chain across the Saltspear.
