How exactly does bastard naming work in Westeros? If Jon Snow really was the child of Ned and Ashara, wouldn’t he be Jon Sand because of the mother’s region or Jon Sand because he was born in Dorne? Or does it go with the dad’s region (but in that case, wouldn’t Brynden Rivers be Brynden Blackfire)? Or is the region of the highest “ranking” parent – so (assuming R+L=J) Jon’s true last name would be either “Waters” (for KL) or “Blackfyre”?

Blackfyre is not the generic Targaryen Bastard name. Just Daemon’s. Hence Brynden and Aegor Rivers, but Daemon Waters pre-legitimation. 

Jon Snow is Jon Snow because he was raised in the North. 

Mya Stone is Mya Stone because she was raised in the Vale.
